Pre-Arrival Health and Waiver Form

Prior to your arrival, we need to collect some necessary health-related information from you! This form will collect your dietary needs, emergency contact info, health insurance information, consent for treatment, and immunization records. Although some of these details may be difficult to track down, in the end it will make for a safer experience in your YWAM missions training program. As always, contact us if you have any questions.
